Use these search options to find facilities and providers in a certain area. You may search for this information two ways.

1. Zip Code and Radius Instant Directory: Use the form to create an Instant Directory of the PPOplus providers in a selected mileage radius within a particular zip code.  For instance, you might want physicians in a 20 mile radius of the zip code in which you live or work. Just enter your zip code into this form and select the "20 miles" option. The Instant Directory will find all PPOplus physicians and facilities within this distance.

2. Zip Code Range: Simply enter a range of zip codes in which the system should search. Your results will give you all physicians and facilities within that range of zip codes. If you want a list of all physicians and facilities in a particular zip code then enter the zip code into both boxes below. For example: 70116 through 70116.
