Search our Directory: This search option allows you to find a doctor or facility in your area
according to name, specialty or location. Statewide Directories may be printed from this
link by selecting Facilities and Physicians and then filling in the state that you are
interested in. This link gives you access to providers in all 50 states if you are accessing
PPOplus for national coverage.
Search by Zip Code: This search option should be used if you need a list of all doctors or
facilities in a certain zip code area. You can specify a zip code with a radius in which to
search or a range of zip codes. This link gives you access to providers in all 50 states
if you are accessing PPOplus for national coverage.
Arkansas Statewide Directory: This link allows you to view and print a PPOplus Statewide
Directory that has already been formatted. This is the quickest way to obtain or print
a statewide directory for Arkansas. However, this link does not allow you to perform
lookups or searches by provider or location. In order to search for a specific provider

This is a directory of facilities and services selected to participate with PPOplus. As with
any directory, changes may occur and PPOplus is not responsible for inadvertent errors or

providers are participating in the PPOplus Preferred Provider (PPO) Network.
PPOplus does not and will not partake in any matters concerning the private and sensitive nature
of the patient-provider relationship. This includes, but is not limited to, all matters concerning
medical decisions, treatments, and the selection of a healthcare provider. Therefore, the final
decision as to which healthcare provider to use and which procedures/treatments are performed lies
with, and is the responsibility of, the insured/patient. In making this decision the insured/patient
must understand that: (1) selection of an out-of-network provider could reduce entitlement to
benefits listed in your health plan; while (2) selection of an in-network provider does not guarantee
that all services recommended or provided by an in-network provider will be covered under your
health plan. Therefore, it is necessary to obtain verification of covered benefits from your plan
provider prior to the rendering of healthcare services
